Job summary

Humana Inc. CenterWell Home Health seeks a Preventive Care LPN to perform in-home visits across the assigned service area, delivering preventive services to patients in their homes.

You will collect specimens, perform vitals, document care in EMR, and collaborate with care teams to improve disease control and reduce complications. Travel is required to reach patients across the Raleigh area.

Qualifications

• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Ability to leverage technology and EMR systems for documentation.
• CPR certification.
• Use your skills to make an impact.

Responsibilities

• Deliver patient-centered, low acuity in-home visits targeting preventive care services.
• Sample collection and management: venipuncture, labeling, and delivering specimens to lab.
• Diagnostic checks: blood pressure measurements.
• Low acuity preventive screenings.
• Document care rendered using EMR for continuity of care.
• Practice infection control principles.
• Facilitate a clean, safe and comfortable environment.
