Job summary

CenterWell Home Health seeks a focused LPN to provide preventive care during in-home visits across our service area. You will perform essential nursing duties, collect specimens, and document care in the EMR to ensure continuity and quality.

Responsibilities include venipuncture, vital signs checks, and strict infection control. A valid NC LPN license, CPR, and reliable transportation are required for this full-time role.

Qualifications

• Active LPN license in North Carolina.
• Valid driver's license and reliable transportation.
• CPR certification.
• Experience with in-home preventive care is a plus.

Responsibilities

• Perform in-home visits for preventive care services.
• Collect samples and manage specimens for lab tests.
• Record care in EMR and ensure continuity of care.
• Observe infection control protocols.
• Travel to patients within assigned service area.
