Job summary

CenterWell Home Health is seeking a Preventive Care LPN to perform in-home visits focused on preventive care services. The role includes travel to patients’ homes, venipuncture for ordered labs, BP checks, and performing low-acuity screenings.

You will document care using EMR systems to ensure continuity of care and adhere to infection control principles. This patient-facing role is part of Humana's TB screening program, with a driver-safety requirement and benefits package.

Qualifications

• Current nursing license
• Valid driver’s license, auto insurance and reliable transportation
• Ability to leverage technology and EMR systems for documentation
• CPR certification

Responsibilities

• Travel to patients’ homes for in-home visits across the service area
• Deliver patient-centered, low-acuity in-home visits focused on preventive care
• Perform venipuncture for ordered tests and ensure proper labeling and transport of specimens
• Conduct diagnostic checks such as blood pressure measurements
• Provide low-acuity preventive screenings
• Document care provided to ensure continuity of care in EMR and other systems
• Maintain infection control to ensure a clean, safe environment
