CenterWell Home Health seeks a Preventive Care Medical Assistant to perform in-home visits delivering preventive services, patient-centered care, and coordinated documentation.

You will collect samples, perform vitals, manage labeling and transport specimens, and ensure infection control while documenting care in EMR. Travel across the service area will be required; a valid driver’s license is essential.

Formación

• A valid driver's license, auto insurance, and reliable transportation are required.
• The ability to leverage technology and EMR systems for documentation.
• Basic Life Support certification is required.
• RMA or CCMA is preferred.

Responsabilidades

• Deliver patient centered, low acuity in-home visits targeting preventive care services.
• Sample collection and management: venipuncture, labeling, and delivering specimens to the lab.
• Diagnostic checks: blood pressure measurements.
• Low acuity preventive screenings.
• Document care rendered to ensure continuity of care using EMR and other systems.
• Practice infection control principles to maintain a safe environment.
