Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to perform In-Home Health Evaluations. The role involves visiting patients in their homes, reviewing medications and medical history, and conducting physical evaluations as part of the health plan's care team.

Physicians will travel locally, work full-time, and support the delivery of comprehensive, personalized health assessments at patients' doors.

Qualifications

• Active, unrestricted medical license in the applicable state(s).
• Multi-state licenses are preferred.
• MD or DO license holders are required to practice in home visits.

Responsibilities

• Provide in-home health evaluations to patients in their residences.
• Engage with patients to bridge gaps in care and build trust.
• Travel locally to patient homes and conduct clinical assessments (including med history review and physical evaluation).
