You will evaluate and treat patients, collaborate with the rehab team, and provide patient-centered care across home and facility settings with a flexible schedule.

Qualifications

• Must hold a Physical Therapy license in the applicable state.
• Master's or Bachelor's degree in Physical Therapy is required.
• Valid driver's license and reliable transportation

Responsibilities

• Provide in-home/outpatient physical therapy to older adults.
• Develop and deliver individualized therapy programs.
• Coordinate with rehabilitation team and caregivers.
• Document patient progress and adjust care plans as needed.
• Ensure safety and promote independence for patients.
