Job summary

Powerback Rehabilitation is seeking a Physical Therapist to provide outpatient and in-home rehabilitation for older adults in Southington, CT and surrounding areas. You will evaluate, treat, and educate patients in their homes or in independent living facilities, designing individualized therapy plans to restore mobility and independence.

Responsibilities include documenting care, coordinating with the rehab team, and adapting plans as patients improve.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice or license eligibility.
• Master's degree in Physical Therapy or Master’s in PT-related field with a Bachelor’s in PT.
• Alternative: Bachelor’s in PT plus 5 years of progressively responsible PT experience.
• Experience with home-based/outpatient rehabilitation is preferred.

Responsibilities

• Provide outpatient rehabilitation services in patient homes or in independent living facilities.
• Develop and deliver individualized physical therapy programs to restore, reinforce and enhance performance.
• Document and communicate clinical findings, goals, and progress to patients and caregivers.
• Coordinate with the rehabilitation team to refer to other disciplines as needed.
• Supervise assistants and students as required by state licensure.
