Job summary

Qualifications

• Graduate from CAPTE-accredited PT program.
• Current license to practice physical therapy in the assigned state.
• Current CPR certification.
• 1+ year of related PT experience for home health care.
• If less than one year of experience, regional clinical leaders' approval and mentoring/residency participation.

Responsibilities

• Provide skilled PT in patients' homes to promote mobility, restore function, and independence.
• Perform interventions, reexaminations at least every 30 days, and discharge planning.
• Collaborate with patients, caregivers, and care team to support recovery and safety.
