• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services in patients' homes to promote mobility and independence.
• Perform reexaminations every 30 days, adjust interventions, and plan discharges.
• Collaborate with patients, caregivers and care team; perform other duties as assigned.
