Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Driver's license with reliable vehicle access.
• Ability to work with diverse patient populations in home settings.

Responsibilities

• Evaluates patients' functional status and PT needs; supports therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes patient responses and reports changes to supervising clinician.
• Treats patients to relieve pain and restore function; maintains standards of practice.
• Instructs patients and caregivers about plan of care and progress toward goals.
• Documents visits and progress notes per policy; completes evaluations and plan of care.
