Job summary

Optimal Care is seeking a licensed Home Health Physical Therapist to help patients regain mobility and independence in their homes across the Midland region. You will deliver one-on-one rehabilitation to support recovery after surgery, injury, or illness while reducing pain and improving function.

You will assess functional abilities and home environments, develop individualized treatment plans, and guide patients and caregivers through interventions.

Qualifications

• Master's or doctoral degree from an accredited Physical Therapy program.
• Current state license as a Physical Therapist.
• Minimum 1 year of experience as a licensed Physical Therapist.
• Reliable transportation, valid driver's license, and insurance.
• Willingness to travel daily for home visits within the service area.

Responsibilities

• Perform one-on-one rehabilitation in patients' homes to improve mobility, strength, and balance.
• Assess functional abilities and home environments to tailor treatment plans.
• Educate patients and caregivers and coordinate with interdisciplinary care team.
