Optimal Care in Michigan seeks a licensed Physical Therapist to deliver one-on-one home health rehabilitation, helping patients regain strength, mobility, and independence after surgery, illness, or injury.

You'll assess functional abilities, develop individualized treatment plans, and guide interventions to improve mobility, balance, strength, and safety in patients' homes. Travel daily within the service area is required, working with an interdisciplinary team.

Qualifications

• Master's or Doctoral PT program is required.
• Current state license as a Physical Therapist.
• Minimum 1 year of licensed PT experience.
• Reliable transportation and valid driver's license.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Provide one-on-one rehabilitation care in patients' homes.
• Assess functional abilities and home environment; develop individualized treatment plans.
• Guide interventions to improve mobility, balance, strength, and safety.
• Educate patients and caregivers; work with interdisciplinary care team.
