Enhabit Home Health & Hospice is seeking a Licensed Physical Therapist to provide in-home care. You will assess patients, design individualized treatment plans, and apply specialty programs to improve mobility and independence.

The role requires a current PT license in the state, at least one year of clinical experience, and a valid driver’s license. You’ll join a collaborative team focused on high-quality home-based care and ongoing professional development.

Qualifications

• Be currently licensed as a Physical Therapist (PT) in the state of employment.
• Minimum of one year of clinical experience preferred.
• Possess knowledge and skill in current therapy practice.
• Valid state driver’s license and automobile liability insurance.

Responsibilities

• Examine and treat patients to improve mobility and function.
• Utilize specialty programs such as Balance and Fall Prevention, Spine Safety, and Joint Replacement programs.
• Collaborate with care teams to support home-based patients.
