Job summary

Jobtailor is seeking a licensed Physical Therapist to deliver in-home care, assess patient needs, and develop rehab plans in collaboration with physicians and care teams.

The role requires CPR certification, a valid state license, and reliable transportation. You will document progress, communicate with physicians and families, and supervise home health aides and PTAs as part of a coordinated care team.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid state driver’s license.
• Personal vehicle liability insurance meeting state minimums.
• One year of experience as a Physical Therapist (preferred).

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient team goals.
• Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit required evaluations, plans, progress notes, and discharge summaries.
