Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients at home, aiming to improve mobility and independence in daily living. You will use physical modalities and coordinate with physicians and the care team.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Help patients improve mobility and independent self-care in daily living.
• Administer skilled care per physician orders and plan of care under supervision.
• Work with Specialty Programs: Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, Total Shoulder Replacement.
