Responsibilities

• Planning and administering prescribed treatment plans in patients’ homes.
• Assessing and documenting physical strengths to support clinical evaluation.
• Developing and implementing rehabilitation programs aligned with physician plans and patient goals.
• Providing clear instructions to PTAs and Home Health Aides, overseeing their work and documentation.
• Documenting patient progress, interventions, and evaluations thoroughly and promptly.
• Reporting patient progress to physicians, care teams, patients, and families, and submitting required documentation.
