Job summary

TEEMA is seeking a Home Health Physical Therapist to perform comprehensive patient assessments, design individualized treatment programs, and deliver skilled restorative therapies in a field/home health setting in Midland, Michigan.

You will educate patients and families, supervise therapy assistants and aides, and monitor care outcomes across diverse clinical settings while collaborating with an interdisciplinary team. Eligible candidates hold a PT degree and a valid state license.

Qualifications

• Education: Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Licensure: Current, unencumbered Physical Therapist license in good standing with the state Board of practice.
• Credentials: Valid driver’s license and automobile insurance.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs, including OASIS assessments, and evaluate care outcomes.
• Develop and implement patient plans of care in collaboration with attending physicians.
• Deliver skilled restorative treatments to reduce pain and restore or maintain function (gait training, modalities, balance/coordination exercises).
• Instruct and supervise home exercise programs and provide home safety instruction to patients and families.
• Direct and supervise Physical Therapist Assistants and aides; participate in team meetings and care coordination.
