Job summary

LiveWell Homecare Agency, LLC is seeking a Home Health Physical Therapist to deliver personalized in-home care, focusing on mobility, strength, and independence. You will plan and administer prescribed PT treatments, assess and document patient progress, and develop rehab plans aligned with physicians.

Supervise PTAs and aides and communicate updates to the care team and family. Required are an unrestricted PT license and CPR certification, plus experience with healthcare professionals.

Qualifications

• Unrestricted Physical Therapy license is required.
• CPR certification is required.
• Experience working with healthcare professionals is preferred.
• At least one year of PT experience is preferred.

Responsibilities

• Plan and administer prescribed physical therapy treatments in patients' homes.
• Assess and document patients' physical abilities.
• Develop rehabilitation programs aligned with physician plans.
• Supervise PTAs and aides and report progress to the care team and family.
