Job summary

AccentCare, Inc. is seeking a dedicated Home Health Physical Therapist to deliver skilled care in patients' homes. You will assess mobility, develop personalized treatment plans, and guide exercises to restore strength, balance, and independence.

This full-time role requires CAPTE-accredited education, state licensure, CPR, and a valid driver’s license with travel to all locations. Benefits include medical, dental, PTO, 401(k) match and professional development opportunities.

Qualifications

• Master’s degree in physical therapy approved by a CAPTE-accredited program.
• Current physical therapist license in the state of operation.
• CPR certification required.
• Licensed driver able to travel to all business locations.

Responsibilities

• Conduct thorough patient evaluations to assess mobility, safety, and therapy needs and establish a clear rehabilitation roadmap.
• Create personalized, evidence-based therapy plans with goals for improving strength, balance, and independence.
• Deliver one-on-one hands-on PT interventions to improve mobility and function.
• Educate patients and families on home exercise programs, safety techniques, and ongoing care strategies.
• Monitor progress, adjust treatment plans, and coordinate with physicians and the care team for safe, effective care.
