Job summary

AccentCare Inc. is seeking a Physical Therapist to provide comprehensive home health evaluations and treatments. You will work as part of an interdisciplinary team, educate patients and families, and tailor plans to each case while documenting progress.

The role requires a CAPTE-accredited PT degree, state licensure, CPR certification, and a willingness to travel to multiple locations. Full-time position with competitive benefits and autonomy in patient care.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program
• Minimum nine months experience as a physical therapist
• Licensed to practice as a physical therapist within the state of operation
• Must possess and maintain CPR certification
• Must be a licensed driver who can travel to all business locations

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
