Job summary

AccentCare, Inc. is seeking a Physical Therapist for Home Health to evaluate and treat patients in a home-care setting. You will work with an interdisciplinary team, educate patients and families, and document care according to standards.

The role includes managing a caseload and traveling to patient locations as needed. Required are a CAPTE-approved Master’s in Physical Therapy, at least nine months of PT experience, state licensure, CPR certification and a valid driver’s license; benefits

Qualifications

• Master’s degree in physical therapy approved by a CAPTE-accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Licensed to practice as a physical therapist in the state of operation.
• Must possess and maintain CPR certification.
• Must be a licensed driver who travels to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic materials per policy.
• Informs the physician of assessment results and interventions before care.
• Develops and documents treatment plans with priorities.
• Educates and counsels patients and families.
• Presents patient information to physicians and keeps team informed.
• Meets productivity standards and manages time effectively.
