Job summary

AccentCare is seeking a Physical Therapist to assess and direct therapy services in a home health setting. You will evaluate therapeutic status and contribute to the total plan of care.

You will instruct patients and families, prepare timely clinical notes, and adhere to the company code of conduct. Requires CAPTE-accredited degree, valid CA license, CPR, and a valid driver’s license with an insured vehicle.

Qualifications

• Master’s degree in Physical Therapy required and CAPTE-accredited.
• Current license to practice as a Physical Therapist in the state of operation.
• CPR certification required; driver’s license and insured vehicle.
• Community/home health experience preferred.
• Must maintain professional standards and comply with agency policies.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and contribute to the total plan of care.
• Direct physical therapy treatment.
• Instruct patients, families and caregivers in use and care of therapeutic appliances.
• Prepare and submit clinical notes in a timely manner.
• Adhere to AccentCare's code of conduct.
