Job summary

AccentCare is seeking a Home Health Physical Therapist to deliver in-home evaluations and therapeutic interventions on weekends. You will assess mobility, design individualized treatment plans, and guide patients through exercises to restore strength and balance, while educating families for ongoing care.

The role emphasizes compassionate, hands-on care and collaboration with the care team to ensure safe, effective rehabilitation in the patient’s home setting.

Qualifications

• Master's degree in physical therapy from a CAPTE-accredited program.
• Current physical therapist license in the state of operation.
• Valid CPR certification is required.
• Must have a valid driver's license and be able to travel to all business locations.

Responsibilities

• Conduct thorough patient evaluations to determine rehab needs and plan.
• Create personalized, evidence-based treatment plans.
• Deliver hands-on therapy to improve mobility, balance, and independence.
• Educate patients and families on home exercise programs and safety.
• Monitor progress and adapt plans in collaboration with physicians.
