Job summary

Travel to patient homes is required, with support for families and caregivers. The role emphasizes compassionate, patient-centered care and clinical excellence as AccentCare leads in aging in place.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.

Responsibilities

• Assess mobility needs and create personalized treatment plans.
• Guide exercises to restore strength, balance, and independence.
• Provide hands-on support in patients' homes and educate patients and families.
