AccentCare is seeking a licensed Physical Therapist to join our Home Health team. You will evaluate and treat patients in their homes, coordinate with an interdisciplinary team, and educate patients and families on care plans.

The role requires a CAPTE-approved PT program degree and a valid state license, plus CPR and driving to multiple locations. The position offers full-time hours with a competitive salary, benefits, and per‑point compensation as applicable.

Qualifications

• Master’s degree in physical therapy from CAPTE-accredited program.
• Minimum nine months experience as a physical therapist.

Responsibilities

• Provide patient evaluations and document treatment per physician orders and policies.
• Develop and implement treatment plans with measurable goals.
• Educate patients and families and coordinate with the interdisciplinary team.
• Meet agency productivity standards and maintain treatment priorities.
