Humana Inc. through CenterWell Home Health seeks a Home Health Physical Therapist to deliver in-home therapy and help patients regain strength and independence, planning, implementing and documenting treatments under the Clinical Manager.

Active PT license and CPR certification are required; teamwork with a care team is essential. Pay per visit ranges from $60 to $83, with full-time hours and comprehensive benefits. This role centers on compassionate, patient-focused rehabilitation in the home.

Qualifications

Current and unrestricted Physical Therapy license.
Current CPR certification.
Experience collaborating with a healthcare team.

Responsibilities

Plan and administer prescribed physical therapy treatments in patients' homes to attain highest physical function.
Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
Develop and implement a rehabilitation program aligned with physician plans and patient goals.
Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
Document patient observations, interventions, and evaluations promptly and thoroughly.
Report patient progress to physician, Clinical Manager, care team, patient, and family; submit evaluations and discharge summaries.
