Job summary

Humana Inc. through CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, assess progress, and guide rehabilitation with a patient-centered approach.

You will work under the Clinical Manager to ensure care plans are executed, progress is tracked, and documentation is thorough and timely. The role requires an unrestricted PT license, CPR certification, and teamwork with healthcare professionals.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
