Job summary

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatments in patients' homes to restore function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Provide clear care instructions to PTAs and Home Health Aides and oversee their documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit evaluations and discharge summaries.
