Job summary

Humana Inc. (CenterWell Home Health) seeks a licensed Physical Therapist to provide in-home care, improving function and independence for patients. You will collaborate with a care team, document progress, and coordinate with physicians and families to meet treatment goals.

This role is full-time, with a competitive pay range and comprehensive benefits. Responsibilities include patient assessment, program development, supervising PTAs and home health aides, and timely documentation of progress

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
