Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home care focused on restoring strength and independence. You will assess patients, design rehab plans, and coordinate with a multidisciplinary team to achieve functional goals.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician plan of treatment.
• Provide clear written care instructions to PTAs and Home Health Aides and oversee their performance.
• Document patient observations, interventions, and evaluations promptly.
• Report progress to physician and care team and submit required documents.
