Job summary

Humana Inc. through CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to provide in-home care, improve mobility and independence, and support patient goals.

Reporting to the Clinical Manager, you will plan and administer prescribed PT, assess strength, and develop rehabilitation programs tailored to each patient. You will supervise PTAs and Home Health Aides, document progress, and communicate with physicians, the care team, patients and families.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Supervise PTAs and Home Health Aides, providing clear written care instructions and oversight.
• Document patient observations, interventions, and progress in a timely manner.
• Report patient progress to physicians, Clinical Manager, care team, patient and family.
