Job summary

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes to improve function.
• Assess and document patients' strengths to guide treatment goals.
• Develop and implement rehab programs aligned with physicians' plans.
• Provide supervision and instructions to PTAs and Home Health Aides.
• Document observations, interventions, and progress promptly.
• Report progress to the physician and care team and manage required records.
