Job summary

CenterWell Home Health, a Humana company, is seeking a licensed Physical Therapist to deliver in-home care under the Clinical Manager, helping patients regain strength, mobility and independence.

You will plan and administer prescribed therapy, assess progress, guide treatment goals, supervise PTAs and aides, document outcomes, and report to the care team. Full-time, 40 hours weekly with a compensation range of $93,000 to $128,000 per year.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
