BrightStar Care of Concord, Lexington, and Woburn is seeking a licensed Physical Therapist for part-time / per diem in-home visits. You will assess clients at home, develop personalized plans, and deliver skilled PT interventions to improve strength, balance, mobility, and safety.

This position offers flexible scheduling, mileage reimbursement, and the chance to support patients in familiar environments. A MA license is required, with a DPT or MPT from an accredited program preferred; new grads

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program required.
• CPR certified (implied)
• 1+ year of experience preferred; new grads welcome.

Responsabilités

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
