Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home PT services part-time or per diem in Massachusetts. You’ll evaluate clients, create care plans, and deliver interventions to improve strength, balance, mobility, and safety in the home.

Expect flexible scheduling, local visits, a mobile EMR for documentation, and mileage reimbursement. Active MA PT license and DPT/MPT required; CPR certified; new grads welcome with 1+ year preferred.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification; 1+ year of experience preferred (new grads welcome).

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
