Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home PT services part-time or per diem. You will assess clients in their homes and design personalized care plans that improve strength, balance, mobility, and safety.

You will educate families, collaborate with a multidisciplinary team, and document care using a mobile EMR while receiving mileage reimbursement. Active MA PT license, DPT or MPT, CPR certification, and vehicle are required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
