Job summary

Active In-Home Therapy is seeking a Home Health Physical Therapist to deliver personalized rehabilitation in patients' homes. The role emphasizes independent practice, collaboration with the healthcare team, and hands-on therapeutic interventions to improve mobility and daily function.

This PRN position offers flexible scheduling and W2 employment, enabling you to cover preferred areas while contributing to high-quality in-home care and patient well-being.

Qualifications

• Current state licensure as a Physical Therapist.
• Home health experience preferred but not required.
• Strong communication and interpersonal abilities.
• Ability to work independently and as part of a healthcare team.
• Proficient with electronic documentation systems.

Responsibilities

• Conduct thorough assessments of patients to determine rehabilitation needs.
• Develop and implement individualized treatment plans with the healthcare team.
• Provide hands-on PT interventions to improve strength, flexibility, balance and coordination.
• Educate patients and families on exercises, mobility aids, and independence strategies.
• Monitor and document patient progress, adjusting plans as needed.
• Collaborate with physicians, nurses and OTs for comprehensive care.
• Maintain accurate patient records.
• Communicate with patients, families, and caregivers to foster a supportive environment.
