Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy; or Master’s in a related field plus Bachelor’s in Physical Therapy; or Bachelor’s degree in Physical Therapy with 5 years of progressive PT experience.

Responsibilities

• Provide in-home outpatient rehabilitation services to older adults.
• Assess patient needs and develop tailored therapy plans.
• Educate patients and caregivers on home exercise programs and safety.
