FOX Rehabilitation is seeking a licensed Physical Therapist in Salisbury, Maryland to deliver one-on-one geriatric care in homes or senior living communities, supporting mobility and independence through the Geriatric House Calls model.

You will evaluate, plan, and provide evidence-based therapy, collaborate with an interdisciplinary team, and may incorporate telehealth for flexible scheduling.

Formación

• Licensed physical therapist with eligibility or active license
• DPT or equivalent degree is required
• Strong EMR proficiency and computer skills
• Current CPR certification

Responsabilidades

• Provide one-on-one PT sessions in homes or senior living settings
• Perform evaluations, develop plans, and deliver evidence-based treatment
• Document in EMR per regulatory standards
• Collaborate with an interdisciplinary team
• Educate patients and caregivers to promote safety and independence
• Deliver telehealth when appropriate
