Job summary

Fox Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care in homes and senior living communities in Wheaton, MD. You will evaluate patients, develop plans of care, and provide evidence-based treatment to improve mobility and independence.

The role offers autonomy, flexible scheduling, and the option to use telehealth where appropriate. You will collaborate with an interdisciplinary team and educate patients and staff to promote safety and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document care in the EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
