Qualifications

• Active Physical Therapy license in the state of practice.
• Current CPR certification.
• Proficiency with EMR systems and documenting patient care.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence; document appropriately in the EMR.
