Job summary

FOX Rehabilitation is seeking licensed Physical Therapists for one-on-one in-home and senior living care across Maryland. You will evaluate, plan, and treat older adults to improve mobility and independence, while coordinating with an interdisciplinary team and utilizing telehealth as appropriate.

This role offers autonomy, flexible scheduling, and opportunities for professional growth within a clinician-led practice dedicated to aging-in-place and high-quality patient outcomes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in-home or in senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging in place.
• Provide telehealth/virtual care when appropriate.
