Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in Salisbury, MD to deliver one-on-one geriatric care in homes and senior living communities, with autonomy to schedule sessions and use telehealth when appropriate.

You will evaluate, plan care, and collaborate with an interdisciplinary team while documenting in FOX's EMR system. This role offers benefits, career growth, and the chance to improve mobility and quality of life for older adults.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and quality of life.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
