Job summary

Powerback Rehabilitation is seeking a licensed Physical Therapist to deliver in-home outpatient rehabilitation for older adults. You will create individualized treatment plans, educate patients and caregivers, and coordinate with the rehabilitation team to maximize independence and function in home and facility settings.

The role requires a PT license, a Master’s degree, reliable transportation, and a flexible, compassionate approach to senior care.

Qualifications

• Must hold a valid Physical Therapy license or license eligibility in the state(s) of practice.
• Master’s degree in Physical Therapy (or equivalent with additional PT degree).
• Driver’s license and reliable transportation to patients’ residences.

Responsibilities

• Provide outpatient rehabilitation in patient homes or in living facilities.
• Develop and deliver individualized PT programs to restore function.
• Educate patients and caregivers on exercises and safety.
• Collaborate with rehab team to select appropriate modalities and referrals.
• Maintain timely documentation and ensure patient care quality.
