Job summary

TheraCare Home Health is hiring a licensed Physical Therapist (PT) for full-time home health services across South Dallas and surrounding areas. The role emphasizes one-on-one, in-home evaluation and treatment to improve mobility, safety, and independence for adults and seniors.

Responsibilities include OASIS assessments, POC development, and education for patients and caregivers, with collaborative care and support from an interdisciplinary team.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Active Texas PT license or eligible Compact licensure.
• Home health experience preferred.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care (POC) based on goals and orders.
• Deliver PT treatment for orthopedic, neurological, cardiac, post-surgical conditions.
• Educate patients and caregivers on home exercise programs and safety.
• Supervise PTAs and aides as required.
