Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health role providing in-home physical therapy to adults and seniors across South Dallas, Lancaster, DeSoto, Duncanville, TX and surrounding areas.

Enjoy flexible scheduling, autonomy in clinical decisions, and a focus on one-on-one patient care in the home setting while helping patients improve mobility and independence. TX PT license required; home health experience preferred.

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active Texas PT license or eligibility for Compact Licensure.
• Home Health experience (1 year required).
• Knowledge of OASIS and home health regulations preferred.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care based on patient goals and physician orders.
• Deliver skilled PT treatment for orthopedic, neurological, cardiac, post-surgical conditions.
• Educate patients and caregivers on home exercise programs and safety.
• Document accurately in the EMR and communicate with care team.
