Job summary

VitalCaring offers a Home Health Physical Therapist role focused on patient-centered in-home care across Texas. You will evaluate functional status, develop plans with physicians, and provide hands-on therapy to improve mobility and independence for patients in their homes.

The position emphasizes autonomy, collaboration with nurses and physicians, and opportunities to supervise PTAs while educating caregivers. Flexible scheduling and ongoing clinical education support professional growth.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed; support transitions of care and discharge planning.
• Document patient care and progress accurately within the EMR; participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
