Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health opportunity in San Antonio and surrounding areas. Provide in-home physical therapy to adult and geriatric patients with autonomy and flexible scheduling.

Responsibilities include evaluations, developing plans of care, one-on-one treatment, caregiver education, and collaboration with interdisciplinary teams, while maintaining compliance with home health regulations and documentation.

Qualifications

• Licensed PT with Texas or compact licensure.
• Home health experience preferred.
• Current CPR certification.

Responsibilities

• Perform home health PT evaluations and OASIS assessments.
• Develop and update Plans of Care based on goals and orders.
• Provide skilled PT treatment for varied conditions in home.
• Educate patients and caregivers on home exercise programs.
• Document accurately in the EMR at the point of care.
• Collaborate with physicians, nurses, and care teams.
