Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health role providing in-home physical therapy to adults and seniors in Grayson and Cooke County, TX. Clinicians can create their own schedules while delivering one-on-one care in the patient's home.

The role emphasizes autonomy, evidence-based practice, and collaboration with an interdisciplinary team to improve mobility, safety, and independence.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Active PT license in Texas or eligible Compact Licensure.
• Home Health experience (1 year recommended).
• Current CPR certification.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop and update individualized plans of care based on goals and physician orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, and post-surgical conditions.
• Educate patients and caregivers on home exercise programs and safety.
