Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health role, delivering in-home PT across Baytown, Highlands, Channelview, and surrounding TX areas. This position offers flexible scheduling, autonomous patient care, and a focus on improving mobility and independence in the home setting.

The ideal candidate will have a Doctorate in Physical Therapy, TX PT licensure, and at least one year of home health experience.

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligible Compact licensure.
• Home Health experience (1 year required).
• CPR certification on file.

Responsibilities

• Perform home health PT evaluations and re-evaluations (OASIS) for adults and geriatric patients.
• Develop and update individualized Plans of Care based on goals and orders.
• Provide PT treatment for orthopedic, neuro, cardiac, post-surgical, and complex conditions.
• Deliver evidence-based interventions focusing on mobility, gait, balance, and independence.
• Educate patients and caregivers on home exercise programs and safety at home.
• Supervise PTAs and home health aides per state regulations.
